Download and Prepare the Gaps-in-Care Spreadsheet (All Measures)
Log into your MDinteractive APM Performance Pathway account and navigate to Review your Data.
2. In the Measure Detail Report, locate the measure you want to update and click View/Edit Patients.
3. Click the green Export Gaps in Care button at the top of the screen. A drop-down menu will appear allowing you to download the list of all patients by NPI (provider) who have care gaps for that measure. Click Export to download the file.
4. Open the downloaded spreadsheet. Each measure includes specific columns that must be updated to close the gap.
5. Update the appropriate fields for the measure (see measure-specific instructions below).
6. Save the updated spreadsheet on your computer.
7. Return to your MDinteractive account and navigate to Upload Files at the top of your homepage.
When uploading, name the file: “Edited Gaps in Care for Measure [Measure #]”
(Example: Edited Gaps in Care for Measure 1).
8. MDinteractive staff will process the corrections.
9. Once processing is complete, MDinteractive will email the ACO to review the updated data.
Measure-Specific Editing Instructions
Below are the exact columns to update for each measure in the Gaps-in-Care spreadsheet.
Measure #1 – Diabetes: Glycemic Status Assessment (>9%)
Update the following fields:
LAB_HEMOGLOBIN_A1C (Column P):
Enter the patient’s most recent A1c value.DATETIME_LAB_DRAW (Column Q):
Enter the date of the A1c test.
Measure #112 – Breast Cancer Screening
Update the following fields:
PREV_MAMMOGRAM (Column P):
Enter Performed* or Not Performed.DT_MAMMO (Column Q):
Enter the date of the last mammogram.
*NOTE: Performed = mammogram within 27 months prior to performance period end; otherwise Not Performed.
Measure #134 – Depression Screening and Follow-Up Plan
Update the following field:
