To export gaps in care, begin by logging into your MDinteractive APM Performance Pathway Account and click on Review your Data.
The Measure Detail Report contains a breakdown of all measure data that has been uploaded to MDinteractive. Click View/Edit Patients next to the measure for which you want to export care gap data.
Next, click the green Export Gaps in Care button. A drop-down menu will appear, allowing you to export a list of all patients by NPI (provider) with care gaps for that measure.
The system also provides the option to export provider outreach letters and patient letters.
- The provider letter includes a list of all patients under that provider or practice who require follow-up care, enabling targeted outreach. These letters are downloaded in a ZIP folder containing individual provider files for easy distribution.
- Patient letters, also provided in ZIP format, are generated to be sent from the provider and include reminders for overdue screenings or services to encourage timely patient follow-up.
