Quality Data Review (ACO)
To view the details of your quality data, begin by logging into your MDinteractive APM Performance Pathway Account and visiting your Dashboard.
Review Data Details at the APM level
To view all quality data uploaded and imported, click on Review your Data.
Measure Details
The Measure Detail Report displays the ACO’s estimated Quality Performance Score (unadjusted)¹, compares it to the 40th percentile Quality Performance Score (76.7%), and indicates whether the Quality Performance Standard was met.
¹ The estimated ACO Quality Performance Score is currently calculated using 2024 benchmarks, as the QPP API has not yet been updated with 2025 benchmark data.
The Measure Detail Report also contains a breakdown of all measure data that has been uploaded to MDinteractive.
Measure Numerators, Denominators, Exceptions, Performance, and Data Completeness
View/Edit Patients
The green “View/Edit Patients” link under each measure ID number opens the Report Patient Record view, which displays a list of individual patients associated with that measure.
Export Patients
From the Report Patient Record view, selecting Export Patients will generate a spreadsheet for further review and analysis.
Show Analytics
Click Show Analytics to view graphs displaying performance rates, data completeness rates, and patient counts — all broken down by practice. Hovering over the bars in each graph reveals detailed information for each practice.

Below the analytics graphs for each measure, you can choose from several summary options to explore your data from different perspectives. Click one of the following to view:
- View NPI Summary
- View TIN Summary
- View TIN Summary for All Measures
- View ZIP Summary
- Map ZIP Summary
- View Provider Type Summary
Review Data Details at the TIN/Practice level
To view all quality data, click Home to return to your APM Performance Pathway Dashboard, then select Review TIN Data.
Measure Details
The Review TIN Data Report contains a breakdown of all measure data that has been uploaded to MDinteractive by practice.
Patient Level Data
Patient data imported for each measure can be viewed by clicking on #1 (Diabetes: Glycemic Status Assessment >9%), #112 Breast Cancer Screening, #134 (Screening for Depression and Follow-Up Plan), or #236 (Controlling High Blood Pressure).

Click Home to return to your APM Performance Pathway Account Dashboard.
