Excel Templates
After adding your MIPS plans, click on Excel Templates to access MDinteractive's quality measure templates.
Choose MIPS Quality Measure
Click here to display the list of quality measures in your account. Then, choose the MIPs quality measure that you want to generate an excel template for.
Sample Template
After selecting the MIPS quality measure, a sample template will appear on the screen.
Download the Excel Template
Click on the Excel button to download the template to your computer and begin data entry.
Data Grid Error Summary – Quick Fix Guide
🔹 Last Column Errors
Issue: Must be “Yes” only.
Fix: Use dropdowns for small edits or correct the Excel file and re-upload. Do not import ineligible (“No” or blank) patients.
🔹 Gender Column
Issue: Must be “Male” or “Female”.
Fix: Correct via dropdown or Excel and re-upload.
🔹 Response Column
Issue: Must match 2025 template exactly.
Fix: Check for typos, spaces, punctuation. Use current-year templates.
🔹 DOS/DOB Columns
Issue: Acceptable formats: 4/15/2025, 04/15/2025, 2025-04-15.
Fix: Reformat and re-upload.
🔹 Age Requirement
Issue: Patient doesn’t meet age criteria.
Fix: Remove from file before upload.
🔹 NPI Column
Issue: Must be 10 digits, no dashes/spaces.
Fix: Reformat and re-upload.
🔹 TIN Column
Issue: Must be 9 digits, no dashes/spaces.
Fix: Reformat and re-upload.
🔹 Software Freezing
Issue: Phantom data causing freeze.
Fix: Click “Clear Grid”, then retry.
🔹 Header Errors
No Header Found: Extra rows or columns.
Header Mismatch: Incorrect headers used.
Fix: Use a clean Excel file with correct template headers, or drag correct headers into place.
💡 Tip
Start with a small batch of patient records. Once clean, scale up to full data uploads.
IMPORTANT: MRN field: This field will not import into your final Quality report if other patient identifiers are included.
Enter and Save
Enter data into the downloaded Excel sheet for all denominator-eligible patients, including both Medicare and non-Medicare patients seen in the current calendar year, and save it. You can then upload your Excel file to MDinteractive under "Upload Files."
Templates that are uploaded successfully will appear in Review Your Data on your dashboard. If the "results" do not appear you either have a formatting issue (see TIPS above), or have not followed the rules of the measure exactly.
