Please follow the instructions on this Practice Fusion link to download QRDA 1 files for the following three quality measures:
- CMS 122 Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9%)
- CMS 2 Preventive Care and Screening: Screening for Depression and Follow-Up Plan
- CMS 165 Controlling High Blood Pressure
- CMS 125 Breast Cancer Screening
The instructions above apply to both QRDA 3 files and QRDA 1 files. Please select the QRDA 1 option in your Practice Fusion menu.
Please also note that Practice Fusion, by default, assigns SNOMED-CT code 185349003 (Encounter for Check-up) as the default code for your office visits. This code is not eligible for CMS 165: Controlling High Blood Pressure, and CMS 122: Diabetes: Hemoglobin A1c (HbA1c) Poor Control (>9%) measures, so you may consider changing the default settings on your EHR to a CPT code like 99213:
